WORKSHOP AGREEMENT
Agreement between Nancy Harvey and _____________________________________________

WORKSHOP TITLE: _________________________________________

It is agreed:

1. Each day of the workshop will be a maximum of 6 hours in length and will begin at _________a.m. each day.

2. The maximum number of students is 12 for a 2 day course; 15 for 3 days and 20 for 4 or 5 day workshops unless otherwise agreed upon.

3. Requesting group will provide:

a) a professional fee of $300.00 per day for instructional services;

b) roundtrip airfare (coach class) from Sacramento, Ca., to the workshop location;

c) lodging/meals;

d) transportation to and from the workshop location and lodging accommodations and the airport.

CANCELLATION
Cancellation of any workshop/lecture must be made at least 6 weeks prior to date scheduled. Payment for services and transportation payable to instructor on the first day of the workshop.
